The physio-anatomical rationale and therapeutic dynamics of dalak (Massage): A comprehensive unani review

Abstract

Introduction: Dalak (massage) is a foundational regimental therapy (Ilaj-bil-Tadbeer) in the Unani medical system, conceptualized as a form of "passive exercise" (Riyazat-i-A'arziya) for the evacuation of morbid humors and restoration of homeostasis.This review aims to provide a specialized analysis of the therapeutic dynamics of Dalak by integrating classical Greco-Arabic medical philosophy with contemporary anatomical and physiological frameworks.Methods: A scoping review was conducted following PRISMA-ScR guidelines.Comprehensive searches of PubMed, Scopus, ScienceDirect, and Google Scholar were performed for 1980-December 2024.Classical Unani texts including Ibn Sina's Al-Qanun fi al-Tibb (1025 CE), Majoosi's Kamil-us-Sana (10th century), and Ibn Rushd's Kitab-al-Kulliyat (12th century) were consulted in published translations.Of 847 records identified, 42 full-text articles were assessed, and 30 sources (12 classical, 18 contemporary) were included.Clinical studies were evaluated using Joanna Briggs Institute checklists.Results: Dalak is established as a regimental intervention that modifies (Ghal rabat) viscous secretions and generates Laf arrat (light heat), facilitating Hazm-e khir (elimination of metabolic end-products) (1-3).Anatomically, Dalak exerts tiered influence through skin (Jild), fascia (Ghilaf-e-Azlat), muscle (Azlat), joints (Mafasil), and neurovascular channels (Majari-e-Dam), achieving therapeutic results via Imala-e-Mawad (diversion of matter).Clinical studies demonstrate: in hemiplegia, massage with Rogan-e-Seer produced significant lower limb functional improvement (p<0.05)(6); with Rogan-e-Qust, mean upper limb Fugl-Meyer gain of 10.55 points (8).Roghaniyat (medicated oils) efficacy depends on temperament (Mizaj) selection (4, 5, 10).Discussion: The layered anatomical impact of Dalak-from cutaneous stimulation to neurovascular modulation provides physiological rationale for applications in stroke rehabilitation, musculoskeletal disorders, and neurological conditions.Synergy between temperament theory and contemporary findings on neuromuscular facilitation validates classical Unani concepts.Safety considerations and contraindications are addressed.Conclusion: Dalak demonstrates physiologically plausible mechanisms and preliminary clinical evidence for movability disorders.Priority research needs: (1) standardized protocols for post-stroke rehabilitation, (2) randomized controlled trials comparing Dalak with conventional physiotherapy, and (3) pharmacokinetic studies of medicated oil transdermal delivery.
