A Rare Pediatric T-Y Distal Humerus Fracture Managed with Delayed Open Reduction: A Case Report

Abstract

Introduction: T-Y type distal humerus fractures are rare intra-articular injuries in the pediatric population. Because of their complexity and potential for long-term functional impairment, accurate diagnosis and anatomical reduction are essential. Case Report: A 5-year-old female child presented with pain and swelling of the right elbow following a fall from stairs. Clinical examination revealed swelling and abrasion without neurovascular deficit. Radiographs and computed tomography confirmed a T-Y type distal humerus fracture with separate medial and lateral condylar fragments. Open reduction and internal fixation using a cross K-wire configuration was performed 6 days after hospital presentation under general anesthesia. The postoperative course was uneventful. K-wires were removed at 6 weeks, followed by physiotherapy. At final follow-up, the patient achieved full, painless range of motion with no complications. Conclusion: This case highlights that even with delayed surgery, meticulous open reduction and stable fixation can result in excellent functional outcomes in rare pediatric T-Y distal humerus fractures.
