A review on the role of Uttarabasti in the management of Vandhyatva (female infertility) with modern correlation

Abstract

Vandhyatva (infertility) is a common gynecological problem affecting many women of reproductive age. In Ayurveda, Vandhyatva is mainly caused by imbalance of Doṣas, especially Vāta, along with defects in Artava (menstrual and ovulatory function), Kṣetra (uterus), Ambu (nutrition), and Bīja (ovum). Among the various treatment methods described in Ayurveda, Uttarabasti plays an important role in the management of Vandhyatva. Uttarabasti is a special Panchakarma procedure in which medicated oil, ghee, or decoction is administered through the vaginal or uterine route. This allows the medicine to act directly on the female reproductive organs. Classical Ayurvedic texts describe the use of Uttarabasti in conditions like Yonivyāpada, Artavaduṣṭi, and Vandhyatva. The procedure helps in normalizing Apāna Vāta, improving uterine health, removing obstruction in reproductive channels, and nourishing the endometrium. It also supports proper ovulation and implantation. This review article presents a simple and clear overview of classical references, indications, drugs used, and the probable mode of action of Uttarabasti in Vandhyatva. Based on available literary and clinical evidence, Uttarabasti can be considered an effective and focused treatment approach in the Ayurvedic management of female infertility.
