Efficacy and safety of k-wires versus drill bits in orthopaedic procedures: A comprehensive review of clinical and biomechanical outcomes

Abstract

Background: In orthopedic surgery, drilling is a critical component for procedures such as fracture fixation, bone grafting, and joint replacement. Conventional drill bits have long been used for creating bone tunnels and pilot holes; however, complications like thermal necrosis, microfractures, and reduced bone integrity remain concerns. Kirschner wires (K-wires) have been introduced as an alternative due to their flexibility, minimal thermal damage, and improved maneuverability. Objective: This review aims to analyze the efficacy, safety, and clinical outcomes of using K-wires compared to traditional drill bits in orthopedic procedures. It focuses on thermal effects, bone integrity, procedural efficiency, and complications. Methods: A systematic review of literature was conducted using PubMed, Scopus, and Google Scholar from 2000 to 2024. The search terms included "K-wires," "Kirschner wires," "drill bits," "thermal necrosis," "fracture fixation," and "bone drilling." Thirty peer-reviewed studies were included, comprising randomized controlled trials (RCTs), cohort studies, biomechanical analyses, and case series. Results ● Thermal Damage: K-wires consistently produce 10–15 °C lower heat than drill bits, reducing thermal necrosis risk. ● Bone Integrity: K-wires reduce microfracture incidence by up to 35% compared to drill bits. ● Maneuverability: Superior in pediatric and osteoporotic bones due to flexibility. ● Efficiency: Procedures are 15–20% faster with K-wires in complex anatomical regions. ● Limitations: K-wires underperform in high-torque drilling required for dense cortical bone. Conclusion: K-wires are a valuable alternative to drill bits, especially in scenarios requiring reduced thermal damage and enhanced maneuverability. However, their torque limitations necessitate careful case selection. Further research should establish standardized guidelines for their use.
