Management of internal root resorption with perforation and periapical cyst in permanent teeth: A case report

Abstract

Adjacent root dentin gradually deteriorates as a result of internal root resorption (IRR), which is started along the root canal wall. Pulpal inflammation triggers the action of clastic cells, which results in IRR. The diagnosis and treatment of internal inflammatory root resorption with perforation linked to an infected periapical cyst are described in this case report. The literature describes the several alternatives for treating the canal resorption defect, such as orthograde and retrograde fillings. Since root canal therapy removes granulation tissue and the blood supply to clastic cells, it is the preferred treatment for IRR. Tissue healing is improved by modern methods such as magnification, sonic and ultrasonic activation of irrigants, and application of bioceramics followed by thermoplasticized heated gutta-percha. The prognosis for the conservative treatment of IRR is favorable even in the case of perforated root walls.
